On behalf of the American Cancer Society (ACS) and the Institute for Alternative Futures – Disparities Reducing Activities (DRA) Project, welcome to the website for the national patient navigator survey.

We appreciate your willingness to participate by completing the survey to help us identify the daily activities and characteristics of patient navigators from across the country. Thanks to the work you do, the field of patient navigation has flourished in the last few years. However, there is still much confusion for many people about the definition and the day-to-day work of the patient navigator. To address this challenge, the Society and DRA Project have joined forces with our academic partners at the University of Illinois at Chicago, Denver Health and the Colorado Patient Navigator Training Program. Together, we hope to use this survey to gather information to help better inform the public at large and the healthcare field about the important work navigators accomplish to help ease the burden on communities and individuals interfacing with the healthcare system.

The survey should take about 15 minutes to complete. If you would prefer a paper copy of the survey to complete and mail back to us, all postage paid please send an email to us. At the end of the current survey you will be asked if you are interested in completing additional surveys over the coming months. We encourage as many people as possible to participate so that we can assure good representation of the type of navigator programs that exist as well more detail on the needs of these types of programs.
